Below you’ll find the documents you need to review and complete as part of our Preferred Stock Offering.
Please begin with the Offering Circular, which explains the terms and risks of the investment. This offering is currently available only to Wisconsin residents, who must complete and return the Investor Questionnaire before proceeding with a Subscription Agreement.
To download documents: Click download. “Add to Cart” all three documents. “Checkout” by filling in your email address and information. And it will then allow you to download the forms. There is NO cost to download these forms.
These documents can be downloaded, printed, and returned along with a check to Frederic Grocery Cooperative, PO Box 112, Frederic WI 54837. A board member will be in touch. Once eligibility is confirmed and the check verified, the Cooperative will issue the official Certificate of Stock to you.
